Please call our Intake Coordinator at 866-511-4325 (HEAL) to arrange for a telephone interview. We will ask you questions about yourself, your background, and current concerns and problems you have. You can anticipate the call to take about 30 minutes. During this time, you can ask any questions you may have about our program. Alternatively, you are also free to complete the Admission Intake Form and mail or fax it to us. We also require completion of the Admission Application Form. This form, in addition to the deposit, is needed in order to hold your spot.
The Oliver-Pyatt Centers believe that a small, intimate environment is most effective for the treatment of eating disorders. Because of this, we treat no more than 8 Comprehensive Day Treatment, and 8 Intensive Outpatient clients at a time. We have a limited number of slots, so it is important to start the admission process early.
What You Will Need to Provide
When you are done speaking with our Intake Coordinator or have completed and returned the Intake Form, the Admission Application Form and deposit, we can plan your date of entry into treatment. We do require a medical evaluation prior to your admission. We will mail, email or fax this document to you or your physician based on your preference. The timing for your physical exam can be planned with assistance from our Intake Coordinator. We will also provide you with a Recovery Statement of Intent form for your completion, which is your commitment to treatment and to the recovery process.

We will ask you questions about yourself, your background, and current concerns and problems you have. As our discussions progress, we will also be interested in speaking with your current treatment providers. We will work with you to arrange this.
